And though the phrase has been uttered countless times in modern adaptations and references, this misquote is almost as old as the books themselves.

It was a popular phrase but someone pointed out that McCoy never actually says it in those words on the show. But then people realized that the original episodes ran about 50 minutes but by the ’80s had been cut down to about 45 minutes for syndication and some lines and scenes that actually existed were cut, and those were the only versions people had seen. According to Memory Alpha, McCoy actually does say “He’s dead, Jim” several times.
